[Apply before discharge from the Hospital at the time of delivery]
I. Birth certificate in Chinese: The original national ID cards of both the father and mother (ARCs or passports for foreign nationals).
II. Birth certificate in English: Passports (or ARCs) of both father and mother.
III. Where to apply: Ward nursing station.
[Application/Reissuance after discharge]
I. For those aged below 20, their legal representative shall prepare the following documents:
(I) Birth certificate in Chinese
Original identity documents of both father and mother.
A household registration transcript of the newborn (within three months; basic personal data and remarks cannot be omitted).
(II) Birth certificate in English
Original identity documents of both father and mother.
A household registration transcript of the newborn (within three months; basic personal data and remarks cannot be omitted).
Birth certificate in Chinese.
The original passports of the father, mother and child (or the household registration transcript in English within three months of all three, which must show their names in English).
II. Those aged 20 and above must prepare the following documents:
(I) Birth certificate in Chinese
The original national ID card of the child.
A household registration transcript of the newborn (within three months; basic personal data and remarks cannot be omitted).
(II) Birth certificate in English
The original national ID card of the child.
A household registration transcript of the newborn (within three months; basic personal data and remarks cannot be omitted).
Birth certificate in Chinese.
The original passports of the father, mother and child (or the household registration transcript in English within three months of all three, which must show their names in English).

Cost
I. Document fee
1. [Apply before discharge from the Hospital at the time of delivery]: Up to 4 copies of the birth certificate in Chinese are free, and an additional
NT$50 will be charged for each additional copy; the first copy of the birth certificate in English is free, and an additional NT$250 will be charged
for each additional copy.
2. [Application/Reissuance after discharge]: The birth certificate in Chinese is NT$50 each, while that in English is NT$250 each.
II. If a certificate of diagnosis or other certificate is issued at an outpatient clinic, in addition to the outpatient consultation charges (including registration fee), an additional document fee will be charged.
Remark
I.The application/reissuance time after discharge is about three to five working days.
II.Aliases or nicknames without official documents will not be accepted.
III. If the content of the original application form is found to be incorrect after pickup and you want to make changes, it will be regarded as a separate
application.
IV. If the legal representative is not the parent of the child, the legal representative should prepare their original identity document and the proof of
the relationship with the patient (household certificate, household registration transcript, court ruling, etc.)
